What a full thyroid panel includes — and when it’s worth asking about
“Ask for a full panel” is the internet’s favorite thyroid advice, usually delivered without explaining what’s in one, why doctors don’t order it routinely, or how to ask so the conversation goes well. All three, here.
The tests, decoded
TSH — the pituitary’s instruction to the thyroid, and the most sensitive routine screen (the thermostat logic: high usually means underactive). Free T4 — the storage-form hormone the thyroid mostly makes, unbound and available for use; with TSH it tells the standard story. Free T3 — the active form, converted from T4 in your tissues. TPO and Tg antibodies — the “why” tests, identifying the autoimmune process behind most US hypothyroidism (see Hashimoto’s vs. hypothyroidism). You may also see total T4/T3 (older-style measurements) and reverse T3 — heavily featured in wellness-industry panels, but not used by mainstream guidelines for diagnosing or managing routine thyroid disease. A fair question for your doctor; a poor reason to buy a $400 direct-to-consumer panel.
The patterns doctors read at a glance
Labs are read as combinations, not single numbers: high TSH + low free T4 is overt hypothyroidism (treatment is standard, settled medicine); high TSH + normal free T4 is the borderline subclinical zone; low TSH + high free hormones is hyperthyroidism, the racing side that deserves prompt follow-up. Context — symptoms, trend, age, pregnancy plans, medications (including high-dose biotin, which can distort thyroid lab results — tell your doctor everything you take) — is the third column no table can print.
Why routine screening starts with TSH alone
Not stinginess — sequencing. TSH is the early-warning gauge that reacts first, so guidelines use it as the entry test and add the others when the TSH result, symptoms, or history justify them. That’s why the productive ask isn’t “run everything” — it’s targeted: free T4 when TSH is off or symptoms persist; antibodies when results are borderline, the family history is loaded, or pregnancy is planned; free T3 in the specific situations where it adds information. Sometimes the honest answer to “would antibody testing help?” is a good-reasons no — and an informed question earns an informative answer either way.
How to ask without friction
The grammar that works is collaborative, not adversarial: “Given my documented symptoms and my mother’s thyroid history, would it be reasonable to check TSH and free T4 — and would antibody testing be useful in my case?” Bring evidence: a two-week symptom log and your dated TSH history from the portal (blank cells in that history are themselves useful — they become the questions). Close with the loop: “Could we note this conversation in my chart, and when should we recheck?” Organized, specific, documented patients are the easiest patients in medicine to serve — and that, far more than any panel’s length, is what gets answers.
