Below that—say, TSH between 4.5 and 10—the decision is more individualized and often starts with retesting rather than immediately starting medication.
The reason for retesting: TSH can fluctuate, and a single result can be misleading. If your TSH was borderline, your doctor may want to confirm the pattern before committing to lifelong levothyroxine. A retest 6–12 weeks later (on the same timing and conditions as the first test) tells you whether the elevation is stable or transient.

What factors does your doctor weigh?

Pregnancy, a strong family history of thyroid disease, or the presence of TPO antibodies (a sign of autoimmune thyroiditis) can shift the calculus toward treatment. It means following the pattern, staying alert to new symptoms, and revisiting the decision if TSH climbs further or if your life circumstances change (pregnancy, new cardiovascular symptoms, etc.). If TSH remains stable and low and you feel well, treatment may never be needed. If TSH keeps rising or symptoms emerge, treatment becomes more clearly indicated.
What about the two ways this can go wrong?
One failure mode is dismissal: a doctor or lab flags a borderline TSH and tells you it's 'nothing' without conversation, leaving you uncertain and unsupported. The other is overtreatment: starting levothyroxine on a single borderline result without retesting or considering your full picture, sometimes leading to unnecessary medication and the risk of overtreatment side effects (palpitations, anxiety, bone loss with higher doses over time).
The right path is in the middle: take the result seriously, retest to confirm the pattern, discuss your symptoms and preferences, and make a shared decision with your doctor about whether a medication trial makes sense for you right now.
